Applying for Institutional Funds

 

A limited amount of institutional funds are available to M.D. and M.P.H. students based on financial need. All M.D. and M.P.H. students are required to submit parental information to be considered for institutional financial aid. The Student Financial Aid Office requires information about your family's financial situation, regardless of your dependency status, so we can make intelligent decisions about the equitable distribution of financial aid resources. If all students were "independent", we would be unable to determine who came from a family that has been historically economically disadvantaged.

 

Students who miss the Financial Aid deadline of March 12, 2014, may be considered for Institutional grant aid, only after awarding all eligible students who honored this deadline, should funds be available. Submitting requested parent income information, after the deadline, does not guarantee that you will be considered for Institutional grant funds.

 

Students accepted for admission after the March 12, 2014, Financial Aid deadline will be granted 14 business days to submit all required documentation. Files completed within the 14 day timeframe will be reviewed to determine eligibility for Institutional grants/scholarships (Need based aid).

 

If you do not wish to be considered for institutional funds and/or feel you will not qualify for these funds, then you should only fill out the "Student's" section of the FAFSA Form. YOU DO NOT NEED TO FILL OUT THE PARENT'S SECTION OF THE FAFSA.

 

An important financial aid concept is the family, to the extent it can contribute, has primary responsibility for financing the cost of the student's education. We consider parental (and spouse's, if applicable) income information on all (includes technically independent students) MD and MPH students.

 

In estimating the amount your parent(s) are able to contribute to your educational expenses, their income, assets, debts, family size and number of family members in college are considered.

 

Students working part-time are expected to contribute toward their educational expenses. Their spouse's income and a portion of any assets including equity in real estate, stocks, bonds and other securities; business and farm equity; cash, and bank account is considered a resource.

Instructions for Applying for Financial Aid

 

Please use the following steps below to guide you through the process.

 

STEP 1: COMPLETE FINANCIAL AID APPLICATION

STEP 2: SELECTIVE SERVICE CERTIFICATION

STEP 3: FREE APPLICATION FOR FEDERAL STUDENT AID (FAFSA)


Any student applying only for the Federal Subsidized Stafford Loan, Federal Unsubsidized Stafford Loan, and the Alternative Loan Program should complete the Student Section of the FAFSA ONLY! Complete the appropriate section of the FAFSA Form at . Our school code is G24821.

 

Expected Family Contribution (EFC): EFC is based on the FAFSA Form computerized analysis of income and asset information that the family provides on the form. It is an estimate of the dollar amount you and your family are expected to contribute toward your education. This information is used to determine your eligibility for institutional financial aid only.

 

Personal Identification Number (PIN): A PIN is needed to apply for student aid. To request a PIN or a replacement PIN, go to . A PIN by mail will take 7 - 10 days. PINs by e-mail will take three days or less. You cannot file the renewal FAFSA until you receive a PIN. You do not need a PIN if you are completing the FAFSA for the first time.

 

Parent Section: This section of the FAFSA Form must be completed only if you wish to be considered for institutional funds (i.e., MSM grants, MSM loans, SDS, LDS, Primary Care Loans).

 

 

STEP 4: FEDERAL INCOME TAX RETURNS

 

You must forward signed and dated copies of your, your spouse's (if married), and your parents'* completed 2013 Federal Income Tax Return, W-2 forms, 1099 forms, and tax schedules.

 

*Only applies to applicants seeking institutional funds for the M.D. and M.P.H. Programs

 

Non-Filer: If any of the persons listed above are required to submit tax returns, have neither filed nor intend to file a 2013 Federal Income Tax Return, they must complete a "Non-filers Certification Form" (W-2 forms and 1099 forms must be submitted to verify all taxable and untaxable earnings, interest income payments, annuities, etc.). The non-filer certification forms may be found within the 2014-2015 Financial Aid application.

 

Federal Tax Return Not Complete: If you, your spouse, and/or your parents have not completed a 2013 U.S. income tax return but will be filing one, it is recommended that you complete your tax return before filling out the appropriate need analysis form. Knowing what will be on your tax return will help you complete the need analysis form accurately.

 

When your need analysis form is compared with the official 2013 IRS Form 1040, or 1040EZ that is filed by you and your family, the financial information must agree. If there are any errors, this will delay your financial aid process and it will take longer to determine your eligibility and receive institutional and federal student aid.



STEP 5:  

NOTE: Complete the Federal Stafford Unsubsidized loan Master Promissory Note (MPN) and the Graduate Entrance counseling session. Do not complete the Graduate Plus loan application and credit check prior to June 2, 2014. Credit checks for this loan expire within 90 days after completion. Submission of any additional credit checks could result in a denial if adverse changes exist on your credit report. Entering students may complete loan applications/credit checks June 2, 2014, through June 13, 2014.

If it is necessary for the Student Fiscal Affairs Office to clarify or verify any information you have provided, other forms requesting additional information will be sent to you. In the event you receive these forms, please complete and return them promptly so that we may determine your eligibility for financial aid.

 

Financial aid files will be considered incomplete until all requested information is received. Students who fail to follow the above instructions and do not submit the required information by the established deadline dates will not receive priority consideration for institutional funds.

 

Funds may not be disbursed prior to registration.
